What your liver actually does all day
Most people can name one job. Filtering. That is not really what it does, and the real list is longer than you would guess.
It banks your energy and hands it back
Your liver stores sugar as glycogen and releases it between meals to keep your blood sugar steady. When the tissue is loaded with fat it stops responding properly to insulin, and that steady release gets less reliable.

Part three
Why this plant, and not the other 40
There is no shortage of things sold for your liver. So it is fair to ask why milk thistle keeps coming up, and whether that is evidence or just habit. Here is the honest version, including the parts that do not flatter it.
It is the most heavily studied of any of them
This is not a plant with 3 small trials behind it. One meta-analysis published in 2025 pooled 55 randomised trials covering 3,545 patients. In liver medicine, that is a serious body of evidence for a botanical.
The fatty liver trials agree with the rest
A separate meta-analysis looked only at fatty liver patients: 26 randomised controlled trials, 2,375 people. Both liver enzymes came down. Cholesterol, triglycerides and LDL came down. HDL went up.
The strongest finding is not a blood test
In the trials where they took actual liver biopsies, people taking silymarin were over 3 times more likely to show improved fatty change. That is tissue under a microscope, not a marker in the blood.
What it actually does in there
Silymarin is the active fraction of the seed. It works as an antioxidant inside the cell, it helps stabilise the cell wall, and it calms the inflammatory signalling that keeps the repair crew on site. In plain terms it supports steps 1 to 3 of the road above.
Plant weight is not a dose
This is the one that catches people out. A tub saying 1,000mg of milk thistle is telling you how much plant went in. It is not telling you how much silymarin came out. Unstandardised extracts vary from batch to batch. Standardised to 80% silymarin means the active content is guaranteed in every capsule. Look for the percentage. If it is not printed, there is a reason.
More is not better
That 2025 meta-analysis broke its results down by dose, and the trials using under 400mg produced bigger reductions in both enzymes than the trials using more. The benefit was never in the biggest number on the front of the tub.
And here is what it will not do
It will not undo established scar tissue. It will not out-run a bad diet.
In the trials it did not show a significant effect in alcohol related liver disease, so if the drinking is still going on, that is the thing to deal with first and no capsule changes that.
And the effect was significant in people with a BMI under 30, and not above it. If there is a lot of weight to lose, the weight is the job. This helps alongside that. Not instead of it.
